Legacy of Grace STO (LOG-STO)’s purpose is to assist qualified families with their tuition expenses by providing financial assistance that is based on family size and household income. 

You are eligible for Iowa STO funds if your income, as found on Line 22 of your IRS 1040 for the 2011 tax year (Line 15 of 1040A) is not more than three times the poverty rate. Below are the qualifying income levels.

Your application is 100% confidential and is qualified by an independent, third-party service. You may transmit this application to this service in one of three ways:

1.     Mail this form and the first page only (not electronic filing statement) showing Line 22 income (Line 15 of 1040 A) of your 2011 Federal Income Tax Return to:

2.     Fax this form and the first page only (not electronic filing statement) showing Line 22 income of your 2011 Federal Income Tax Return to Christian School Tuition Assistance Service - fax 815-589-2439.

3.     Scan in this form and the first page only (not electronic filing statement) showing Line 22 income of your 2011 Federal Income Tax Return and email to: ctd@christianstas.com

The deadline for submitting the forms is April 30, 2012. All forms must be forwarded or postmarked by that date to be considered.
